Your appointment with Mr Booth will take place at either of his consulting rooms in Box Hill or Wantirna. Telehealth consultations are also available.
Mr Booth also offers consultations with uninsured patients for the purpose of their diagnosis and treatment plan.
Face to face consultations attract a fee which is reimbursed, in part from Medicare.
Our reception team can process your claim immediately and the rebate will be deposited directly back into your bank account.
*The consultation fee will be advised to you at the time of booking your appointment.
Patient consultations are available on Monday’s, Tuesday’s & Friday’s.
Both face to face and telehealth appointments can be made by calling the rooms (03) 9897 4922 during business hours:
Monday to Thursday: 9:00am – 5:00pm
Fridays: 9:00am – 3:00pm
YOUR FIRST APPOINTMENT
At your first appointment, Mr Booth will take a thorough history and perform a detailed examination of your affected joint or injury.
This is to determine the impact your condition is having on your everyday life.
You may be referred for further X-rays, an MRI or other tests to confirm the diagnosis.
Whilst the procedures Mr Booth performs are highly specialised, not all patients’ problems require operative solutions.
Mr Booth will discuss all possible solutions and openly communicate the various outcomes.
WHAT TO BRING
- The referral letter from your GP. See REFERRALS page.
- Prior to your appointment please complete the Patient Registration Form and submit online or print and bring with you on your appointment day. DOWNLOAD THE FORM.
- X-Rays, MRI scans, CT scans, Ultrasound or other images you may have including the imaging reports.
- If you have seen a physiotherapist or other allied health professional please bring a progress letter from their practice also.
- Your Medicare card and your Private Health Insurance details.
- Wear clothes that will allow easy access to the affected joint or injury during the clinical examination.
*If your condition or injury is work-related, please bring your workers compensation details including your claim number, insurer, and case manager’s contact details.
